All-in-one platform that's perfect for my small private practice!
My business is very part-time on the side, and I didn't have time or resources for some of the other options that are on the market. TheraNest gave me everything I needed to run my practice easily, efficiently, and professionally.
Pros
It's simple to use and has everything you need to make your small private practice run. The customer support is AMAZING! I've never had a question or concern that wasn't quickly resolved by their great team.
Cons
I would like to see the TeleHealth screen be bigger. This is a work in progress since this is a newly added feature.

Very good value for money
I very much like TheraNest. While there are glitches, for example, the 2FA isn't properly working yet, the tech support team responds fast and is motivated to figure things out and implement new features.
Pros
Price for value. Good features. Fairly intuitive. Good tech support.
Cons
Difficult to set up divorced parents for one child as system is not well set up to handle that; complicated. Document signing can be difficult when individual forms need signature by a client. Did not like the telehealth option as can only be used with clients, not prospects.

For a lot of headaches use TheraNest
Do NOT use theranest unless you want major headaches, have extra time added to your plate just to use the software, have a year of and have a lot of money to waste. Their unnecessary upgrades they have decided to make makes working with theranest unbearable.
Pros
Ease of charting and making appointments. That part is typically ok - unless the system is down - which happens way more than it should.
Cons
Not sure where to begin! Theranest is an extremely complicated system and there is a big large learning curve for all users - even just clinicians that make appointments and chart. It is extremely cumbersome for billing and insurance work. Back buttons have to be used all the time - at least 3 times to update client insurance claims. Look at invoice, enter what the client owes, save. Back button - write off the money the insurance won't pay, save, back button, charge the client what they owe, back button, send the client a statement (their "automatic" receipts don't show what the date of service is). I really need someone else trained on the billing part of the system and I cannot even imagine the extra time I would have to take to train someone and the mistakes that would be made. There is way too much room for user mistakes when using the theranest billing system. I pay a lot extra for their billing portion to use their clearing house, yet I am consistently having to go to each payor's websites and figure out what is going on with various claims when the Era's come back to theranest - I should say "if" they come back to theranest. Today their ERA processing suddenly stopped working and now I get to spend hours and hours updating clients billing charts manually for the week! We have 6-7 clinicians and it already takes me 2 full working dates to reconcile accounts with theranest. when things are working "properly". They claim to be very proud of their billing system - as they have "added" features to make the billing process quicker- (which I found their "upgrade3" doesn't even pull the correct amount a client owes and it needs to be manually fixed anyway and I have to manually write off money insurance won't pay). Oh by the way - they upped their monthly fee because they added this "upgrade" that doesn't work. Their updates at times causes problems elsewhere in the system - which means something else like updating a chart for example or simply saving notes you have written stops working. God forbid you don't remember to save everything every 2 seconds or all will be lost. When adding a diagnosis to a clients charge - you have to save it 2x in two different areas or it doesn't save! Customer service is "available" but expect to wait for days before they reply back to you - usually asking questions you have already told them in your request for help email. When you send a message back reiterating what you already said - there is never a quick reply. So any problems with the software could literally take a week to fix. I actually got someone today on chat - which is the very first time in the 6 years this has ever happened - and then had to spend literally one hour chatting about our ERA issue today because he failed to explain anything in any clear way - only directed me to some form and because between each line of chat - it took 2 minutes for the message to even get to each other. (oh yeah, I forgot to mention that the system is super slow at times-not my computer - it happens to all of us). They are constantly changing things with the software that are an unnecessary waste and cause a lot of confusion and time for the user because all the sudden something is different. For example, for some reason only they know, they stopped allowing the same email to be used in different client accounts (we work with children). We see many sibling groups and use their parents email on their chart to send statements or reminders, etc. Now we can't use the same email. So again - extra time and money had to be spent for my office manager to call these families and ask for different emails. Parents like information to go to one email. Many parents weren't happy about this. Who checks 3 different emails everyday? Customer service's response was "We recommend you have the parents create emails for their children to access the data, and also should help them distinguish and filter data between their kids if they have multiple". Um, ok. Basically - they will not go back to allowing us to use one parent email for his or her kids. After years and years of headaches and so much extra work theranest adds, just to use the expensive software, that was the straw that broke the camels back. We are searching for a new system - stat. Have not done it before now because the thought of switching all clients over and the learning curve of a new system was too much. BUT - now we are done.

TheraNest--Not the best, not the worse.
My overall experience with TheraNest is okay--it's a love-hate relationship. The forms are not as dynamic as they seem compared to other EHRs, and you cannot send client documents other than intake/consent forms. There is no option to separate child and parent profiles for consent--you have to either put the parent's email in the child's profile or provide paper consent forms to the parent. You cannot link parent-child relationships and customize client details to accommodate your practice.
Pros
The support team and support portal have been very helpful and timely.
Cons
TheraNest's "dynamic" forms are not as dynamic compared to other EHR for therapists. It takes so long to create the most basic form in TheraNest and you cannot duplicate questions. You cannot edit the client details tab to customize it to your practice nor can you send clients documents in TheraNest to sign or view outside of consent forms. I have used other EHRs at other practices, and TheraNest seems to have the least features and easability.

I wish there were premade template library in the document section. It is a little wonky trying to create documents and then it is trial/error to get them to where you need which is very time consuming. I am still going through my documents and needing to change small things that I didn't catch. That is one feature that I do miss from Simple Practice- they had an amazing library of document templates, assessments (PCL5, health assessments, etc.,).

Sole practitioners not a priority
In the beginning the experience was great. Good service, training, and responsiveness. Over past year this has changed and I was told larger accounts get prioritized. I am looking for a new medical billing service but will likely continue using the EHR part for now.
Pros
Inputting progress notes is quick and easy. Navigating adding clients, storing records, and reliability of site working is the best thing about TheraNest
Cons
The billing service is expensive for what you get. You will spend a lot of time tracking payments and following up on your own. Your invoice records will not be kept in a timely way and you won’t be alerted if invoices aren’t paid. If not paid you won’t know why unless you spend hours contacting insurers.

Theranest lies about their customer service
From time to time, I was able to break through to their "upper management" and was told they really care about me and my clients and my ability to deliver therapy to those in need. This too turned out to be a falsity.
The decision to cut me out was born of my holding them to their word about live customer service.
I have requested to speak to the business owner, but so far, the coward has not had the necessary honor to call a fellow business person to discuss this travesty.
I am considering legal action against this company to recoup the money it will cost me to take down my data from Theranest and use another competitive software.
THEY ARE POISON. Stay away from them.
Pros
It's a comprehensive program that handles most aspects of the claim processing we all need in a busy practice.
They SAY they offer live customer service when a user runs into questions about how to use the program. That is why I chose them over Simple Practice, which offers NO live customer service.
The reality is when you are in need of an answer on how to use an aspect of the program, you must leave a message on a voice mail.
They will call you back when it's convenient for them to do so - and that's never when I need an answer.
I do all of my claims submissions myself and I'm a fairly intelligent and computer literate user. This is a complex, intricate software with may nuances in use.
My time is limited, and so it doesn't work for me to have to wait for an answer until it's convenient for them to call me back.
Cons
Simply put, they lie about their live customer service.
Their training videos are convoluted, not well-produced and hard to follow in terms of content.
I spent countless hours and significant money to make the transfer over to Theranest, and when I held them to account for their promised and not delivered live customer service they CANCELLED MY ACCOUNT!
This has caused me undue stress and dollars...and they have been very callous and offered no-compromise position. I complained too much, and therefore, they got rid of me! Nice!

Prevents you from leaving the EHR
Thumbs down. Too long to respond to tech support. Ignored me for a week and still doing so to open up my deletion period and generally do not have a fix for the answer so, you just cope with it.
Pros
Theranest had ease of navigation inside their system for the most part.
Cons
If you do not use their clearing house, they charge you extra and then start messing up your EDI and billing. They will NOT allow you to manage your charts in a timely manner when trying to leave their system. I have been trying to leave their system since August and it is now October. Because they are making it difficult to leave, it is forcing me to pay for a system at a high price that I am not using. For example, you have to delete everything one at a time, and then they only let you delete 3 people a day. So, they are making bank because I cannot leave their system from their lock down of MY CHARTS. Imagine when you have almost a thousand or more people to exit the system how this impacts you financially as a business owner. So, essentially, I am paying for 2 EHRs.

User Friendly EHR

I have used Theranest for over five years since starting my solo private practice. The ease of use has been outstanding. I have everything I need to keep organized records, appointments and invoices all in one place. I transitioned to providing Telehealth seamlessly with Theranest during the Covid pandemic as well.

Overall, I enjoy this platform over others I have tried. It is easy to use without being overly simplistic in design.

Theranest as an EMR for Pediatric OT
Our overall experience with Theranest is that it meets a need for a decent price. Nothing great. Nothing horrible.
Pros
The pricing structure and informative reporting are what I like best about Theranest.
Cons
I would appreciate more flexibility with storing logistics information for an entire family using one parent email and tracking of all customized documentation (particularly Pediatric Evaluations, Plans of Care, and treatment session documentation).

More than a 5 star!!!
I have read reviews on products before and wondered if they were from real people! I am a real therapist in a small private practice that was growing faster than I could keep up. I tried Theranest after trying to buy another very expensive program, and I am so glad that I found Theranest. It is the perfect software storage for the small business owner. I appreciate that the cost is in direct relation to the amount of business I have. I appreciate that it does not charge by the month- by the therapist. It is solely based on client numbers. I have been using theranest now for one month and I am so pleased. All of my questions have been answered immediately by customer service and all of the answers were an easy solve! Thank you Theranest for a great - affordable- easy to use- easy to change- product! It is fantastic!!!

Terrible product.
Pros
The looks are easy enough on the surface. But even from the there the functionality is poor.
Cons
The billing side is terrible. They won't let you post a zero pay payment, like a deductible, in an easy way. You need to post the payment, then find the patient, do the codes, and do the adjustment. And if there is secondary insurance, the user has to mark it to bill. These should all be done for the user.

Not great for larger practices
Most everything has already been said. It's been like many up and coming technology companies - not a lot of great customer support; communicating through email is frustrating (because they don't read completely what your issues are, etc.); and lots of kinks to be worked out. I also feel as if their "updates" are focused on completely superficial and peripheral things and not on the core of the software.
Pros
The therapists on staff at my agency love the software. With only a few complaints, they find it easy to use and obviously are happy to not have paper files any longer. We have used this software since July 2018. I also appreciate that TheraNest offers a discount for nonprofits, which our counseling agency is a nonprofit. This helps with my budgeting.
Cons
The billing and reporting of this software is lacking in many ways, from the lack of robust reporting options to very elementary and clunky billing system. I would NEVER recommend this product for a group practice. Literally, we asked them early on - due to their statements being very difficult to read - if they had any accounting professional associated with the development of the software, and the answer was no. I have a CPA who reconciles our books every month and it's been a nightmare trying to reconcile TheraNest. Again, it might be good for a solo practitioner or for someone who is less worried about reconciling finances.

TheraNest for Small Group Private Practice
Overall, I have truly valued using theranest for my work. It has been a wonderful system for practice management that I can do virtually everything I need to for my clinical practice. Once you’re familiar with the system, it is so easy to navigate through and complete the work in a timely manner. Although not without its frustrations, Theranest still has room to grow and develop. But I’m happy to wait along for the changes they have made.
Pros
TheraNest offers our practice the ability to effectively manage a caseload from all different sizes, and enables us to maintain confidentiality of patient information even across different users. I love that I don’t have to worry about someone else seeing the notes or appointment dates other than those who should.
TheraNest’s template for progress notes, intakes, and treatment plans are super helpful in keeping me organized and making sure that I do not miss any information that needs to be documented.
Additionally, I appreciate the software’s consistent updates that take into consideration feedback received from users like me. I was especially appreciative of their addition of separate gender identity and pronoun lines in the client details section.
Cons
Learning the software is difficult - wherein we notice a lot of people making errors in setting up appointments or being able to navigate through the site.
We also find that few people use TheraNest the same way across the board, which creates some inconsistencies for us on billing and case management.
Additionally, I have noticed minor fixes that could help to change the overall experience. For example, there’s no option for a user to cancel/change multiple appointments at one time; e.g., when a therapist is going out of town for a week. Also, there is no option to maintain a history of client demographic information, once you update the information (or the client does) it is gone.
Lastly, there are seemingly no options to enter notes or addendums to notes that are signed or attached to a billed invoice. As a clinician, we notice mistakes at times and need to include additional comments like follow-ups with collaborative providers, client homework, etc. that would be best attached to the session note.

Motivated & Personalized Customer Service, User-friendly EHR that is likely to continue to...
Overall pretty great product and service and seems to be a company that is focused on development and growth of the product and customer service experience. I've listed the things I like the least but even after studying other options, haven't been compelled to switch. I'd recommend them.

store all client data in organized, accessible fashion
can subscribe to small cost subscription (lower amount of active clients)

Integrated client schedule with insurance billing

Cons
clients can complete paperwork electronically but there are some flaws--can't require client signature field so have to do work around; ideally this would offer more official signatures as often as needed (this is a big liability issue)
secure messaging included but these conversations can't be added to the cart through a click and also copy and paste function is deactivated. secure messaging also not available to with non clients to share PHI for example with a medical provider.
Some issues with superbills showing a balance in a confusing way; we can't reflect discounts which creates liability issues, we can't send these through secure message easily (only email)
I've heard too many complaints about the video platform so am not using it (or paying for it) at this time
cumbersome to upload documents to the chart (I haven't figured out how to skip step of saving to my computer first)
Simple Practice seems to have them beat in two areas:
In TheraNest, Can't require client to enter their cc information when they register electronically
In TheraNest, Client self-scheduling is not user friendly so I don't offer it

Good value for money, has some serious functionality deficiencies
Mostly positive. You're pretty much on your own to figure it out, although they do have a nice library of info and videos. The videos are typically very short, single issue focused, and include a transcript. Their longer videos are chaptered, so it's easy to skip sections that you don't need. We don't take insurance, so we don't use any of that functionality, so I can't say how well that works. Looks like it would make tracking insurance billing/payments much easier.
Pros
Fairly easy to use. Nicely integrated EHR, telehealth, credit card processing. Using as an all-in-one package for our practice management. Generally good training videos and knowledge base.
Cons
Some critical functionality is lacking, or requires a work-around. (You cannot create a repeating telehealth appointment. WTF?) The way couples/families are implemented is a bit wonky, with differences from individual appointments. Telehealth is limited to a max of 6 people (including therapist). Seeing as we (and I assume most eveyone else) have been using Telehealth exclusively for all appointments for the last 1 1/2 years, I'm really bothered by the telehealth scheduling issue.

What I like most about TheraNest is the simplicity and functionality. I love that TheraNest actively implements user feedback to enhance my experience. Having a chat feature where I can connect with a representative is very efficient.
Cons
I would like more multiple choice features when it comes to filling out a MSE or a progress note. I would like the feature to print a progress note without a treatment plan attached, I would like to have the option to redact information before printing, and I would like for the little note icon on the calendars weekly page to be a different color when I sign a note as opposed to not signing the note.

Ruined my Billing
It's been awful. Their services are so terrible, the customer service is none existance, it almost forced me to close my business because of the billing issues Theranest created by using there in house billing. Worse, THEY DO NOT CARE! There is no customer service, only a chat bot and a random email that say "a ticket has been created". No one ever helps you to resolve your issue. Now I'm missing thousnads of dollars in claims and can't find a way to correct it. I've been in private practice for 10 years and always used office ally and always managed my own billing. I should not have left office ally. DO NOT TRUST YOUR FINANCES AND BUSINESS AT THERANEST!

It was a find product 5 years ago and worked primarily ok until Theranest offered for me to use their integrated billing system. I thought I was saving myself energy, time and money. The opposite has happened. I've had to switch EHRs because they took over my system and completely lost all of my bills, I have no way to track them, they are gone. They will not call me to help. I've lost thousands of dollars because of their billing system.
Cons
They offered integrated and managed billing. It never worked, they never assisted me with help, and now I am missing thousands in claims. I can not figure out how to fix it. Everything was find using Office Ally, and then I switched to Apex at their recommendation. I'm so upset. It has created hours and months of distress and they only feign a response. I'm constantly told "I'll create a ticket for this" and then they never get back to me, or deny accountability for there even being a problem. They are like working with another insurance company. I will never recommend this EHR. I will actively advise against it.

Small Private Practice Review
Overall minus some minor changes, the experience has been positive.
Pros
I love how it is easy to set my practice forms the way I want them and it converts to a PDF file as well. Also, the price is reasonable for an EHR and starting a private practice.
Cons
The tiered pricing for client volume and seems like there should be a cap on it. It would be nice for group notes, that I use for couples counseling, to give me the option to use the note I designed instead of the one built in.

Good just needs IOS

Originally I liked the ease of charting and scheduling with appointment reminders… but the system is often down and being upgraded.
Cons
Managed billing is a scam. Customer service is non existent. If you are a sole provider then take your money elsewhere or hire someone to assist you for less money overall.

Excellent. I have been using TheraNest since 2014. The program originally served my needs as a solo practitioner in search of an efficient paperless practice management tool from the beginning and has only been getting better since.
Pros
Ease of use for my paperless solo psychotherapy practice.Critically important for scheduling, patients records, patient histories, therapy notes, and billing.
Cons
There are still a few residual button-labeling, semantic inconsistency, and page-targeting issues that I think need to be resolved for the program to perform at its very best from a user-interface and user-experience point of view. That said, improvements are continuously being made by the Development Team. Note that the user interface has been recently revised and refreshed for dramatically improved appearance and ease of navigation.

User friendly start to finish.
TheraNest is capable of providing the manager and practitioners ways to individualize their workflows from charting notes to communicating with clients and other work mates. Training to use the product flows well and your can use the self-help options for finding the answer to most user questions. When you are unable to find the answer yourself or you have a glitch, customer service is courteous, patient, and solves the issue.
Pros
Ease of set up, daily use, billing management with Optum and Medicaid.

I would like to have complete billing management with all of my insurance products, esp. Anthem/BCBS

Works great!
Pros
Theranest works great for my 2-person private therapy practice. It's easy-to-use, simple, and the price is right. I'd definitely recommend Theranest to a colleague.
Cons
There are some little design things that I wish read more easily (like the main calendar) and places where the payment collection could be easier, but there's nothing that's seriously difficult about using Theranest.

Excellent Documentation System!
I certainly think that it is a great system overall and has great features that enhance the writing experience as well as the potential to further improve as opportunities are given to make suggestions towards improvement.
Pros
I like that I can change note types and being able to make appointments and leave comments in the appointment log section. It is good that addendums can be left related to the note already created.

At times data does not save properly such as signatures or appointment status such as appointment "Kept" for the day of service.

Ideal platform for my small sole prop therapy practice
TheraNest allows me to securely store client paperwork, including consents and progress notes. The calendar is accessible to my clients in their client portals, which allows for easy scheduling.
Pros
I started my business as a part-time venture while I transitioned from full-time employment with a larger company. I liked being able to pay a lower-tiered amount, depending on the number of clients I had enrolled. As my practice has grown, it has been easy to adjust my subscription to reflect my business' changing needs. The telehealth platform is easy to use. It does not have a whiteboard function, although it does allow for screen sharing. There are times when the platform is unreliable, so I have a backup video platform account to use in these instances.I do not bill insurance, although accepting credit cards and logging cash payments into the system is seamless, with easy to access invoices and superbills.
Cons
The video platform (while it has been recently updated), could use some improvement.

Ease to use, and the best Customer Service
I have accurate records and I have several options for my notes. I love the new feature of the electronic portal for client paperwork. The appt calendar works best on the computer but I can make good changes on my phone.

I have loved watching this program develop and grow in the last 10 years. I began Theranest when they were first starting and it was pretty bare bones. Which is what I needed as a new business owner so that I could grow with the program. At that time, I really wanted exact notes that would reflect exactly what happened in the sessions and Theranest provided that place. It has grown however to a much better system and a much better process. I know that I do not use a lot of the process because I have an outside biller. But all the parts I do use are the best and I have tried several.
Cons
The cost of anything is always a killer for a small business. The cost however has been fair for the service that you get.

Review of Theranest
Theranest provides everything that is needed for my practice. It has been simple to learn and use throughout the years. I appreciate all the updates that are made to accommodate the various practices, the access to Knowledge Base and the quick response from customer support when I've needed additional help.
Pros
I've been working with Theranest for almost 10 years. Theranest software keeps my client files HIPAA compliant, which is crucial in the mental health/medical field; I have never had a problem. Once set up, billing is very simple. If I'm having a difficult time with billing, records , program, etc, customer support is easy to access. I appreciate the Knowledge Base that has been developed as well as how Theranest has consistently improved the software or made adjustments based on their customers recommendations.
Cons
I'm a very small practice (3 individuals). I don't like having to pay extra for changes that I would like to make i.e. using Wiley notes. I understand how Theranest makes it's money but for something that doesn't require ongoing support, some things should just be another option ie. choosing between Standard note or SOAP note.I also feel the percentage for assistant billing (following up with insurance providers for unpaid invoices) is very high, considering once you figure out the problem and I begin to be reimbursed for services and no further assistance from Theranest staff is required; I am still expected to provide you with a percentage of what I'm billing. Perhaps just charging a one-time fee.

In hospital and community mental healthcare jobs I have used more than 10 EHRs over the years. TheraNest has it figured out. It's sleek design makes it enjoyable and easy to use--you can customize what you want to see and what you don't. Scheduling and billing are a breeze, tele-health built in for a nominal fee, treatment plans and goals of therapy helps also for a small fee. And the analytic reports are so helpful in running my business. I love everything about it.
Cons
The only criticism I have is in the pop up window when trying to build a recurring schedule in which there is a date conflict--the pop up message is only that there is a conflict--it does not show you the date that needs to be resolved. That takes a little time in narrowing down, but not a huge deal.

One company was used by our clinic for patient scheduling, patient notification (by text message), billing, and notes, which created a significant risk of human error and failed to meet the unique requirements of our practise. The majority of our workplace demands are met by TheraNest in a single, integrated package. scheduling, reminders for appointments, updates on progress, billing, and ERA (eventually). I looked over the ERA in advance, and it appears that money will be applied to billed services effectively. TheraNest has my highest recommendation for other behavioural health practitioners since I am so pleased with it.
Pros
Years of data were accurately and quickly imported into TheraNest from another system. They provided excellent customer service and were upfront about the shift. Our office tested several other well-known applications, but they all had serious drawbacks. After using a competing programme for more than 30 years, we desired and required TheraNest, which is fully featured. We have a lot of enthusiasm for Theranest. The programme is new to our office, but so far so good. Excellent client service is provided. They constantly introduce new features, including those that people specifically ask for. The initial learning curve is manageable, and our staff members mastered it in a matter of days rather than weeks. The planning, pt.The agenda, calendar, and appointment notifications are thoughtfully designed and perfectly integrated. It took some getting accustomed to, but the initial intake note and progress notes work quite well—DX codes are simple to enter and updated. The progress notes are signed and cosigned as needed and are highly professional. I'm aware that this sounds like a sponsored commercial, but after investing a lot of time, money, and effort in testing out various products, I can honestly say that Theranest gave me the biggest boost. By January 2019, we hoped to have completed the switch from our present system to Theranest. We anticipate billing our first full month of receivables for November 2018 ahead of schedule.

At first, the costs were intimidating. But now that I know more about TheraNest, I can definitely see the potential for a really effective, time-saving, integrated system that would be well worth the monthly cost. Also, there is flexibility in monthly billing based on caseload (growing or reducing at the user's choosing), and it looks reasonable.
