> Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) are two types of virtual care services that are used to manage the health of patients with chronic conditions. 

RemetricHealth offers both services, together or independently. We provide physician groups, hospitals, home health agencies, community health centers and payers with remote, real-time biometric and therapeutic monitoring, management tools and remote care services conducted by our nursing staff to assess patients remotely between visits.

These solutions help improve outcomes and may reduce the number of acute events, ER visits and hospital readmissions.

REMOTE PATIENT MONITORING SERVICES

Your patients use select medical devices at home,
including a blood pressure monitor, weight scale,
pulse oximeter, and/or glucometer. Readings are
automatically sent to RemetricHealth’s secure and
HIPAA-compliant web portal.

A nurse assigned to your patients will monitor vital
signs as part of the approved care plan. The nurse
offers additional support to help the patient in
taking measurements, while reviewing trends and
any symptom changes.

CHRONIC CARE MANAGEMENT SERVICES

Chronic Care Management (CCM) is designed to
manage chronic conditions at home, between
office visits. Our clinical team provides patient
enrollment, training, biometric and therapeutic
monitoring, and ongoing service of the program on
behalf of your practice.

The program yields improved patient outcomes
and satisfaction, a more efficient way to manage the
growing number of patients with chronic disease,
and a new and reoccurring revenue stream.
