It has several licenses to choose from if you prefer to use it hosted. It is based on Bootstrap 3 and has a sophisticated theme editor, which needs to be used for adjusting themes according to your wishes. It has a nice Stats feature that provides a quick analysis of collected data in real-time. Although, it is mostly helpful when you use default question types without extensive modifications with JavaScript.
